Payment Options

  • Self Pay

    A detailed receipt “superbill” will be provided for your lactation visit. You submit it to your insurance, and they might reimburse you.

    Reimbursement is at the discretion of your insurance provider and is not guaranteed

  • United HealthCare (UHC)

THINGS FOR YOU TO REMEMBER

Credit card is REQUIRED for ALL bookings

Any charges will be charged to card on file prior to consultation

All in home consultations have a 50 convenience fee not covered by insurance. 25 travel fee for appointments over 30 miles

There is a 15 minute grace period for all in person appointments. Following that time appointment will be cancelled & require rescheduling

No- show will be charged 50 fee charged to card on file

Appointments cancelled prior to 48 hours will be refunded deposit.

Appointments cancelled 24 hours prior forfeit deposit

You’re on the road to getting the support you need. Check your email (including SPAM). Please review and complete all consents and intake paperwork withing 24 hours of appointment.

Due to HIPPA compliance appointments that have not completed required consents prior to appointment are subject to cancellation
